The facility failed to ensure that medications were administered according to physician orders for one resident with a G-tube. The resident was readmitted with diagnoses including COPD with acute exacerbation, hyperlipidemia, dysphagia, major depressive disorder, and altered mental status. The resident’s MDS showed a BIMS score of 15, indicating cognitive intactness, and the care plan identified the resident as NPO with tube feeding and aspiration precautions due to alteration in nutrition related to the G-tube and significant weight loss concerns. An APRN later confirmed that the resident should have been receiving medications via the G-tube as ordered.
