Summary

The facility failed to follow its menu for residents on therapeutic diets, including a resident on a mechanical soft diet and a resident on a carbohydrate-controlled diet. The menu spread sheets provided by the Certified Dietary Manager specified that the mechanical soft diet was to provide ground meats for breakfast, lunch, and dinner, but during dining room and tray line observations, the resident was served roast beef cut into pieces or chunks and diced or chopped chicken instead of ground meat. The resident had diagnoses including dysphagia, type 2 diabetes mellitus, dementia, and hypertension, and had a BIMS score of 10, indicating moderately impaired cognition. A second resident with diagnoses including type 2 diabetes mellitus, end stage renal disease, and above-knee left leg amputation was ordered a CHO controlled, high protein renal diet with regular texture and thin consistency. Observation of the resident's breakfast tray showed 2 slices of wheat bread, scrambled eggs with peppers, oatmeal, bacon, apple juice, and milk, and the lunch tray included 2 rolls, chicken cacciatore with penne, strawberries with whipped cream, and cranberry juice. The resident stated he received what everyone else did and was not aware that he was on a special diet. Record review and interviews showed the facility's diet orders in the EHR did not match the food service supplier's diet spreadsheets. The Certified Dietary Manager stated the facility did not have a renal diet on the spreadsheet and used a high-protein diet instead, and he acknowledged that the breakfast and lunch served to the resident contained too many carbohydrates and that rolls were substituted for breadsticks. The dietician stated she was aware the EHR diets did not match the supplier spreadsheets and agreed there should be no guessing about what residents were receiving. The CDM also stated he had not been shown the diets and had made assumptions about which menu matched the renal diet order.
