Summary

During one continuous observation, a resident’s call light was not answered for 25 minutes while staff walked past the room or did not stop to check on the resident. During another observation, a resident who needed help getting out of bed waited while a CNA said she was waiting on a nurse to assist; after repeated returns to the room, the CNA ultimately told the resident she would have to complete care in bed because another staff member could not be obtained. Grievances from the prior months also documented call light delays of up to several hours and showers being cancelled or not completed because of staffing concerns. Staff interviews acknowledged that the facility often needed more help on all shifts, that call-offs affected coverage, and that some showers could not be completed because there was not enough staff.
