Failure to Transcribe and Implement Antibiotic Order and 72-Hour Monitoring After Change of Condition

Lighthouse Healthcare CenterLos Angeles, California Survey Completed on 04-02-2026

Summary

The deficiency involves the facility’s failure to implement its Change of Condition Notification policy for a resident who developed a swollen right jaw related to a wisdom tooth. The resident, who had GERD and moderate cognitive impairment and required set-up/clean-up assistance for eating and oral hygiene, had a Change of Condition (COC) assessment completed on 3/5/2026 indicating the jaw swelling and that the physician was notified. Orders were received to refer the resident to a dentist and to monitor the resident for 72 hours for acute changes. A progress note by the Social Services Assistant documented that the resident was referred to the dentist and that the call was transferred to an LVN for orders from the dentist. A subsequent progress note by the LVN documented communication with the dentist, who stated that orders for oral antibiotics would be faxed to the facility as soon as possible. The dentist’s written order, dated 3/5/2026, directed administration of amoxicillin 500 mg every eight hours for seven days, and the Director of Social Services stated this order was faxed to the facility that same afternoon. However, there was no documentation in the resident’s medical record that the antibiotic order was transcribed or carried out. The RN responsible for care planning after a change of condition stated she was not made aware of the change of condition and therefore did not develop a care plan for monitoring or interventions, and 72-hour monitoring was not completed. Another RN confirmed there was no documentation of the antibiotic order being transcribed or implemented and no documentation of 72-hour monitoring related to the change of condition. The facility’s Change of Condition Notification policy required the licensed nurse to document pertinent details of the incident, update the care plan to reflect the resident’s status, and document each shift for at least 72 hours, which was not done in this case.

Staff later stated the night nurse was notified, while the resident said BG was not checked again until breakfast.

Failure to Provide Ordered Wound Care and Aspiration Precautions
D
F0684 F684: Provide appropriate treatment and care according to orders, resident’s preferences and goals.
Short Summary

Failure to provide ordered wound care and aspiration precautions: A resident returned from a dermatology procedure with biopsy-site dressings and the chart lacked documentation of assessment or wound care, while the dressings remained in place and undated during observations. The same resident also had dysphagia with a FEES showing silent penetration with straw sips and an order for no straws, yet was observed drinking water from a cup with a straw; the SLP and DON confirmed the no-straw precaution and expectation to follow physician orders.
