The facility failed to remove one resident’s left-hand mitten restraint in accordance with the physician’s order. Resident 8 was admitted and later readmitted with diagnoses including acute respiratory failure with hypoxia, tracheostomy, gastrostomy, and traumatic subdural hemorrhage. The resident’s MDS dated 3/20/2026 indicated severely impaired cognitive skills for daily decision making, and the resident required extensive assistance with toileting hygiene, bathing, dressing, and bed mobility. The facility policy on restraint holidays stated that holidays from restraints provide residents who are not appropriate for restraint reduction with a temporary break from restraints.
