Summary

The facility failed to ensure the call light was within reach for one resident in his room, contrary to the facility policy titled, Call Lights: Accessibility and Timely Response. Resident 127 was admitted and later readmitted with diagnoses including generalized muscle weakness, hydrocephalus, type 2 diabetes mellitus, major depressive disorder, hypertension, prostate cancer, failure to thrive, CVA, and dementia. The Quarterly MDS dated 2/26/2026 indicated severe cognitive impairment with significant limitations in memory, orientation, and attention, and that the resident required substantial to maximal assistance with footwear, dressing, toileting, and bathing. The MDS also noted clear speech and that the resident’s preferred language was Farsi, while the admission record identified Persian as the primary language. During observation on 3/2/2026, the resident was sitting in his wheelchair in his room with the call light placed behind him on the bedside dresser. Staff interviews confirmed the call light was not positioned in front of the resident or within easy reach. A CNA stated the resident did not know how to use the call light and that it was kept behind him so it was out in the open, while an LVN stated it should always be placed in front of the resident and within easy reach. The DON later stated call lights are meant for residents to request staff help and should always be in the resident’s dominant hand for easy access, and acknowledged that for this resident the call light was behind him and inaccessible.
