Infection Control Lapses During Tube Feeding and EBP Care

Summary

Proper infection prevention and control practices were not followed for two sampled residents. Resident 23, who was admitted with diagnoses including hemiplegia, hemiparesis following a non-traumatic intracerebral hemorrhage affecting the left dominant side, and dysphagia, was observed receiving a tube feeding bolus with an unlabeled syringe. During the observation, the LN confirmed the syringe was not labeled with a date and stated that tube feeding syringes should be labeled with a date. The IP later stated that tube feeding syringes should be labeled with a date to prevent use beyond 24 hours, which could increase the risk of infection. The facility policy for enteral feedings indicated that administration sets for open-system enteral feedings should be changed at least every 24 hours or as specified by the manufacturer. The facility also failed to follow Enhanced Barrier Precautions for direct care activities involving two residents. Resident 19, who was admitted with pneumonia, had a care plan directing staff to monitor adherence to EBP guidelines, especially gown and gloves during high-contact activities. During observation, a CNA replaced Resident 19's soiled dressing covering a tracheostomy scar without wearing a gown and confirmed he was not wearing one. Resident 23 also had a care plan directing adherence to EBP guidelines, and during the tube feeding bolus observation, the LN did not wear a gown while providing care and acknowledged that she should have worn one. The DON stated that staff were expected to follow EBP guidelines during direct care, and the facility policy identified device care and wound care as high-contact activities requiring gown and gloves.
