On February 27, 2026, the Administrator and Director of Nursing were interviewed and the facility's QAPI program was reviewed. The Administrator stated the QAPI committee included the Administrator, DON, Medical Director, Infection Preventionist, Director of Staff Development, and department heads. The Administrator also stated he was aware of late responses to residents' call lights from residents' council meetings. A review of the facility document titled Quality Assurance and Performance Improvement (QAPI) Plan, dated August 2017, showed the facility's stated policy to maintain an ongoing, systematic, proactive, facility-wide, data-driven process to measure, assess, carry out, and improve resident care, outcomes, and safety.
