Summary

The facility failed to implement infection prevention and control practices for two residents. Resident 171 was admitted with acute bronchitis due to rhinovirus and had an order for Enhanced Droplet Precaution. During observation, EDP signage outside the room indicated staff entering the room were required to wear an N95 mask, face shield, gown, and gloves, and a PPE cart was present. CNA 3 was observed in the room walking away from the resident’s bed and cleaning the vital signs machine while wearing a gown, gloves, and surgical mask, but not wearing the required N95 or face shield. CNA 3 confirmed he was unaware of the required PPE for EDP rooms. The Infection Preventionist stated Resident 171 was on EDP for respiratory symptoms associated with positive test results for rhinovirus, enterovirus, and MRSA of the nose, and that all staff were to wear gloves, gowns, face shield, and an N95 when entering the room. Resident 16 had a diagnosis including neuromuscular dysfunction of the bladder and was on Enhanced Barrier Precautions for an indwelling catheter. During observation, EBP signage outside the room indicated staff participating in transferring were required to wear gown and gloves, and a PPE cart was present. CNA 4 was observed assisting another CNA with transferring Resident 16 from bed to wheelchair with a Hoyer lift without wearing a gown or gloves. CNA 4 stated she did not have to wear the required PPE because Resident 16 was not exposed. The Infection Preventionist stated Resident 16 was on EBP for an indwelling catheter device and that staff assisting in high-contact activities, including transferring, were required to wear PPE such as gown, gloves, and mask. The facility policy stated EBP required gown and glove use during high-contact resident care activities, including transferring.
