Failure to Provide Vision-Related Assistive Supports

Good Shepherd Health Care Center Of Santa MonicaSanta Monica, California Survey Completed on 02-25-2026

Summary

The facility failed to ensure that Resident 29 was provided with assistive devices to maintain independence in ADLs and support communication with staff. Resident 29 was admitted with bilateral legal blindness, dementia, and metabolic encephalopathy. The MDS dated 01/23/2026 indicated the resident required substantial to maximal assistance for ADLs and partial assistance for eating. During observation on 2/23/2026 at 8:13 a.m., Resident 29 was lying in bed at the lowest position with the call light on the floor, and there was no oxygen concentrator at bedside or any visual sign indicating that the resident was legally blind. The resident’s head was resting on a white pillow, and no other cushions were observed. During interviews, CNA 2 stated that staff must do everything for Resident 29 because he is blind and cannot select his own clothes or feed himself, and that he walks with assistance and should not be left alone. CNA 3 stated that staff cannot always stay with Resident 29, that there should be a communication binder and a visual board at bedside, and that the resident cannot use the call light in his room because he does not know how to use it. CNA 3 also stated that Resident 29 is continent of bladder and that staff must borrow a walker from therapy to take him to the restroom. The SSD stated that the resident is seen once a year by the optometrist, that the optometrist does not make recommendations, and that the facility staff cannot implement interventions to assist with ADLs. The SSD also stated that vision is important because it can affect ADLs and that there should be a visual sign in the room to notify others of the resident’s impairment.
