The schedules reviewed did not appear to show excessive call-ins/no-shows, and the facility assessment documented staffing ratios that were compared to the Quarter 4 weekend schedules with no discrepancies noted or low weekend staffing as reported to CMS. During interviews, HR-G stated that corporate offices submit the PBJ data and that HR-G sends information to corporate, which handles the submission; HR-G also stated that corporate had not raised concerns with staff. When asked about the Quarter 4 PBJ staffing issue, HR-G stated that after reviewing the schedules with corporate, they were not sure why the facility was flagging for low weekend staffing and could not explain the discrepancy.
