Summary

The facility failed to ensure that a Level II PASARR referral was made for one resident who had been reviewed for PASRR screening. The resident was admitted to the facility after acute inpatient hospital care, and the Level 1 PASRR completed on 10/21/2025 marked Section IA for SMI indicators, including mood disorders and anxiety. The resident’s Level 1 PASRR should have been sent after 30 days of admission, but it was not.
