Incomplete Bed Assistive Device Entrapment Assessment

Summary

The facility failed to thoroughly assess the use of a bed system assistive device for one of five residents reviewed for accident hazards, Resident 102. The facility policy on Proper Use of Mobility Enabler Device(s) stated that a person-centered approach must be used when determining the use of mobility enabler devices, and that the resident assessment must evaluate risks such as falls, entrapment, and other injuries, as well as assess the bed, mattress, and rail for entrapment risk. The policy also referenced FDA guidance identifying entrapment zones in hospital bed systems, including zone six, the space between the end of the rail and the side edge of the headboard or footboard. Resident 102 was admitted on February 26, 2026, and had an active physician order dated February 27, 2026, for a right HALO bar to promote independence with bed mobility. A physical therapy assessment documented the resident’s use of a HALO device on the right side of the bed. Observation of the resident’s room on March 18, 2026, showed a bed with a right-sided circular assistive device, a headboard, and a footboard. Review of a Test Results Worksheet dated February 27, 2026, showed staff failed to assess a potential entrapment risk for zone six and incorrectly documented an assistive device on the left side of the bed when none was present. The resident confirmed that she had only ever had the assistive device mounted on the right side of her bed.
