Failure to Implement Infection Control During Gastrointestinal Outbreak

Summary

The facility failed to adhere to proper infection control practices during a gastrointestinal outbreak affecting residents on four care units. Facility policy required surveillance for infections, implementation of control measures, and use of transmission-based precautions for residents with documented or suspected communicable disease. The policy also directed staff to notify the physician, implement precautions, post signage, provide PPE, and limit resident transport when TBP isolation was needed. Facility infection control documentation showed nine residents on the D unit with loose stools, nausea, and vomiting, followed by nine additional residents on the A, B, C, and D units with similar symptoms. Review of clinical records for the residents involved in the outbreak found no evidence that transmission-based precautions were implemented to prevent spread of the gastrointestinal illness. Resident R9 continued to have foul loose stools on 3/29/26, and stool was to be obtained and evaluated for C. diff. Observations of R9 on 3/30/26, 3/31/26, and 4/01/26 showed no transmission-based precautions in place. During an interview on 3/30/26, Resident R33 was observed resting in bed with ginger ale and saltine crackers and stated, "I threw up a couple times and just feel awful," with no transmission-based precautions maintained during the observation. An LPN on the C unit reported leaving work due to nausea and stated that several residents on the unit were not feeling well with a gastrointestinal illness. Another LPN on the C unit later stated there was no knowledge or nurse-to-nurse report of residents on the unit with a gastrointestinal illness. The Infection Control RN confirmed the facility was not following proper infection control practices and failed to recognize additional symptomatic residents or implement transmission-based precautions to mitigate spread across the A, B, C, and D units. The NHA also confirmed the facility lacked evidence of surveillance and implementation of infection control measures to prevent further gastrointestinal illness throughout the facility.
