The facility failed to ensure complete competency documentation for four of five nursing staff reviewed, including Employees E5, E7, E8, and E10. The facility policy titled "Staffing, Sufficient and Competent Nursing" defines competency as a measurable pattern of knowledge, skills, abilities, behaviors, and other characteristics needed to perform work roles successfully, and states that competencies are intended to measure the knowledge, skills, and abilities necessary for staff to perform their roles effectively. During an interview on March 5, the Educator, Employee E6, confirmed that the competency records were incomplete for Employees E5, E7, E8, and E10, and that validation of the competencies could not be verified because the required tasks were not checked as either Met or Not Met.
