Summary

The Nursing Home Administrator (NHA) and Director of Nursing (DON) failed to effectively manage the facility to ensure that a newly admitted resident with End-Stage Renal Disease and a kidney transplant history received a life-sustaining medication. The resident was admitted with an order to continue Tacrolimus 1.5 mg by mouth every 12 hours, a critical immunosuppressant needed to prevent organ rejection. Review of the Medication Administration Record showed that no Tacrolimus order was entered on admission, and the resident did not receive the medication from February 27 through March 3, 2026, missing eight doses. Facility policies required nursing staff to obtain a complete medication history, reconcile medications with hospital discharge summaries, prior MARs, and physician orders, and ensure critical medications were administered promptly. The resident’s laboratory results on March 3, 2026 showed a critically low Tacrolimus level of less than 2.0 ng/mL, below the therapeutic range.
