On 1/13/26, Resident 15 stated that extra doses of trazodone may have been given and that the resident had been unable to do anything but sleep for two days, missing a physical therapy session that was important to the resident. On 1/14/26, a family member stated the resident had been in tears and said being so tired prevented functioning, and the family member reported being told by a CMA that trazodone had been increased by 50 mg on 1/7/26 and 1/8/26. Staff interviews showed the CMA noticed the duplicate order and that doses had been administered, notified the nurse on duty, and later saw the order remained unchanged. An LPN stated she had a pending trazodone order entered by an RN trainee without knowing there was an additional verification process, and the RN trainee stated she had not realized she was creating an order and had not paid attention to the resident or medication information. The DNS stated the order was an error and should have been deleted, not confirmed, and that nurses should verify orders when implemented.

The DON reviewed the MAR and stated it was not accurate because staff had different ideas of what restlessness was, and stated the resident should have the correct manifestation of anxiety reflected in the physician order and MAR. The facility policy stated psychotropic medications are to be used only when a practitioner determines the medication is appropriate to treat the resident’s specific, diagnosed, and documented condition.
