Summary

The facility failed to review and revise care plans to reflect the current status of 3 of 13 sampled residents, including Resident #3, Resident #5, and Resident #22. For Resident #3, the medical record showed a diagnosis of Parkinson's Disease and a physician order dated 10/18/23 that was discontinued on 11/15/24 for a resting hand splint to the left hand daily, on in the morning and off at bedtime as a nursing measure for left hand contracture. The current care plan still stated that the resident needed assistance with ADLs and to wear a brace to the left hand and arm on in the morning and off at bedtime, but it was not updated to reflect discontinuation of the left hand and arm splint. An administrative nurse confirmed the care plan had not been updated. For Resident #5, the current physician orders identified Lasix for kidney disease, but the care plan did not include problems, goals, or interventions related to diuretic use. For Resident #22, diagnoses included macular degeneration and bilateral cataracts, and a physician consult report stated that eye screening and low vision testing were completed, lamp use was explained, yellow fitover sunglasses were ordered, and the resident should be encouraged to look just to the left of things for the best vision. The care plan stated the resident was at risk for decreased ADLs due to weakness and impaired vision, but it did not include the specific interventions recommended by the physician or the resident's use of a flashlight. During observation, the resident was seen in the activity room with a four-wheeled walker and two flashlights on the walker seat, and the resident voiced concern about severe vision loss.
