Summary

The facility failed to ensure an accurate Preadmission Screening and Resident Review (PASRR) Level 1 screening was completed for one resident at admission. Record review showed that this resident’s face sheet listed diagnoses of unspecified dementia, moderate, with agitation, and bipolar disorder, unspecified, at the time of admission. However, the PASRR Level 1 form completed for this resident indicated that the resident did not have a mental illness diagnosis.
