On 1/22/26, the posted NHRCSR showed a census of 117, but the staffing sheet later provided by the LNHA reflected a census of 118. On 1/27/26, the posted NHRCSR showed a census of 116, while the LNHA provided a staffing sheet with a census of 118. During interviews, the Receptionist stated the Staffing Coordinator was responsible for posting the NHRCSR around 8 AM, while the Staffing Coordinator stated she ran the schedule, completed the count, entered the information into the state report, printed it, and posted it in the lobby around 7 to 7:30 AM, sometimes asking a Supervisor to post it if she was out or late. On 1/29/26, the Staffing Coordinator stated she sent out staffing with the census the night before and changed it in the morning, and that the posting for those days should have been accurate. The LNHA stated the staffing reporting was done the night before, that two discharges did not come in after midnight, and that the posted NHRCSR was not changed before 7 AM. The facility's Staffing Policy dated 9/1/25 stated the facility provides sufficient numbers of staff with the skills and competency necessary to provide care and services for all residents, but it did not include information about the regulations for posting, and management stated there were no other staffing policies.

On 6/3/2026, staffing information dated 6/4/2026 was posted at the 100 Hall nursing station near the off-hours facility entrance. During interview, the DON confirmed the staffing information posted was dated 6/4/2026 and stated the scheduler would prepare the staffing information sheet before leaving and corrections would be made the next day. The DON also stated the staffing sheet should not be posted in the front and should be placed behind the current day until the new shift starts. Administration stated the staff coordinator should not post the staffing information the day before and should post the current staffing information for the day. The Staffing Coordinator stated she had been putting the staffing information out before leaving at the end of the day for the following day and did not notice the census was incorrect.

During interview, NA-A verified the posting was dated 5/26/26 and stated it was important for families and visitors to see how many staff were working. SC-B stated she changed the nurse staffing hours on Thursdays for the coming week but did not update the posted sheets, and instead made the changes in the computer after they occurred when she returned on Mondays. A review of the next week's schedules showed the census as 24, and one schedule dated 5/30/26 had zeros for all staff listed with a census of 24. The DON verified the nurse staff posting should be posted daily so anyone could see how many residents were in the facility and how many staff were working, and stated it was important so families could have confidence there was enough staff to take care of their family members.
