Survey Results Not Readily Accessible
Summary
The facility failed to post the State of New Jersey inspection results in an area readily accessible to residents, families, and the public, and failed to ensure reports of surveys, certifications, complaint investigations from the prior 3 years, and any available plan of correction were available for review. On 2/20/26, the surveyor asked staff at the reception desk and on the Heritage Manor unit where the survey results were located, and staff were unable to identify the location without asking others. The surveyor was directed to a white binder labeled Annual Long Term Care Survey in a bookshelf in the Hearth Area, but the binder contained only the most recent recertification survey dated 10/8/24 and a Focus Infection Control survey dated 1/17/24, with no other survey results filed. The DON confirmed that the binder contained only those two survey dates and stated she would get back to the surveyor regarding the facility policy. The LNHA later stated the facility had no policy regarding posting survey results and believed only the most recent recertification survey was required. The LNHA also stated she was not aware that the binder should include at least 3 years of surveys, including complaint surveys. The surveyor was informed that the survey results were not easily accessible at the reception desk or on the unit, and the facility history reviewed by the survey team showed additional surveys, including a recertification survey, an FIC survey, and complaint surveys, that were not filed in the survey binders.
