The facility failed to ensure that Certified Nurse Aides (CNAs) received at least 12 hours of mandatory in-service training for 5 of 5 CNAs reviewed. During review of the facility’s in-service education records for the 2024 to 2025 year, the surveyor found that the education consisted of a sign-off list and did not include an hourly audit of the training provided. The facility was unable to produce documentation showing the number of hours each CNA had been trained or any performance evaluations tied to the in-service education. Interviews with facility leadership confirmed the lack of documentation and tracking. The DON stated there was no system in place to document hourly training and/or performance evaluations and could only provide checklists for the CNAs. The HRD confirmed there were no competencies for staff and that the CNA education was presented on a trifold bulletin board with no documentation of hourly training. The ADON also confirmed there was no system to document hourly training and could only provide checklists. The LNHA acknowledged that the education in the building was not being done correctly. The facility policy required annual in-service training of no less than 12 hours per employment year and included training on cognitive impairment, dementia management, and resident abuse prevention.

Review of the personnel records for NA Employee E2 and NA Employee E3 showed hire dates of 3/12/24 and 4/5/24, respectively, and their files did not contain documentation showing they had received the required minimum 12 hours of in-service training during their respective yearly periods. During an interview, Human Resource Employee E9 confirmed that the facility failed to ensure these two nurse aides received the required yearly in-service education.

GNA #28's file contained documentation that the 12-hour yearly training was completed on 5/20/24, but there was no documentation that the training had been completed again since 2024. During the review on 5/15/26, the Human Resources Manager stated there was no documentation in the personnel files or computer showing when the last 12-hour training occurred and said there could be documentation in the previous DON's office, but was not sure. The Nursing Home Administrator later stated he could not find documentation in the previous DON's office related to any 12-hour training for the GNAs.
