Summary

The facility failed to maintain complete, accurate, and readily accessible resident medical records. Surveyors were unable to obtain Activities of Daily Living logs for a resident with dementia, restlessness, and agitation, despite the resident having a BIMS score of 8 out of 15. The facility also could not provide declination sheets or monitoring documentation for residents receiving methadone until the end of January 2026, even though multiple residents were on methadone and surveyors requested those records during the investigation. The facility also failed to provide complete and accurately documented 1:1 observation records for two residents. Both residents had BIMS scores of 15 out of 15 and diagnoses that included depression and chronic pain for one resident, and hemiplegia and hemiparesis following cerebral infarction, type 2 diabetes mellitus, and low back pain for the other. Surveyors found missing or incomplete entries on the 1:1 logs, and the records did not match the facility’s policy or the EMR/eTAR/eMAR documentation. Staff interviewed during the survey stated that documentation should be complete and accurate, that blanks on the 1:1 record were improper, and that the logs should be available and current. The facility was also unable to provide a personal belongings inventory for a resident who was re-admitted to the facility. That resident had diagnoses including opioid abuse, neuropathy, and mixed hyperlipidemia, and a BIMS score of 12 out of 15. Surveyors requested the inventory because the resident was alleged to have brought pills back from the hospital and was observed inhaling them through the nostril. The DON stated that belongings are inventoried at admission, but she was unable to provide the inventory log requested by surveyors.
