Summary

The facility failed to ensure a safe discharge for a resident who had been admitted with endocarditis and a history of IV drug use. The resident also had diagnoses of alcohol use disorder and opioid use disorder documented in the discharge paperwork from the hospital. The resident’s care plan addressed antibiotic therapy, pain medication therapy, endocarditis, infection risk, and risk for PICC dislodgement, but it did not include any focus, goals, or interventions for discharge planning. The resident’s progress notes documented repeated PICC complications, including up to 9 dislodgements of the line. On the day of discharge, the resident was discharged home with the PICC still in place after refusing removal because of concern for possible excess bleeding.
