Failure to Follow Hand Hygiene and Glove Change Practices During Wound Care

Summary

The facility failed to ensure staff used infection control practices to prevent cross contamination during wound care for 2 residents. The facility’s hand hygiene policy stated healthcare personnel should perform hand hygiene before moving from a soiled body site to a clean body site on the same patient, after contact with blood, body fluids, or contaminated surfaces, immediately after glove removal, and when changing gloves during patient care if moving between body sites. One resident had dementia, hemiplegia, hemiparesis following cerebral infarction, and morbid obesity, and had a Stage II pressure injury to the right gluteal fold. The wound care order directed staff to remove dressings, clean the area, apply Calcium Alginate and skin prep, and cover with bordered gauze. During observed wound care, an LPN failed to perform hand hygiene between glove changes and failed to discard the Calcium Alginate dressing after it fell from the wound site and touched the resident’s bed linen, becoming contaminated. The LPN confirmed both actions were breaches of infection control, and the ADON also confirmed the dressing should have been discarded and hand hygiene performed between glove changes. A second resident had peripheral vascular disease, MRSA carrier or suspected carrier status, protein calorie malnutrition, and post bariatric surgery, and had wound care orders for both lower extremities. The order directed staff to wash and dry the legs, apply Calcium Alginate with Vaseline to open areas on the right leg, cover with Vaseline gauze and ABD pads, and continue betadine to both legs. During observed wound care, the same LPN failed to change gloves and use hand sanitizer between cleaning the right and left legs and failed to change gloves and use hand sanitizer between applying betadine and applying dressings to both legs. The LPN confirmed these actions were a breach of infection control, and the ADON confirmed that moving from one wounded body site to another without changing gloves or using hand sanitizer was a breach of infection control.
