Infection Control Failures With Hand Hygiene and Respiratory Equipment

Summary

The facility failed to complete hand hygiene before donning new gloves and between glove changes during resident care. During an observation of a transfer for one resident, a nursing assistant performed hand hygiene, donned PPE, and began the transfer process, but after removing gloves and sanitizing the lift, did not perform hand hygiene before putting on new gloves. A medical record clerk entered the room, did not perform hand hygiene, and put on gown and gloves while spotting the lift. In a separate observation during wound care, a unit director-registered nurse removed gloves multiple times and donned new gloves without performing hand hygiene between glove changes while handling soiled dressings, labeling a dressing, discarding a soiled brief, and handling a new brief. The facility also failed to follow respiratory equipment infection control practices for a resident with COPD who was ordered oxygen and had an intervention to provide oxygen as ordered and change and maintain oxygen tubing as ordered. The resident’s oxygen cannula and tubing were ordered to be changed every seven days for infection prevention/standard practice, but observations showed the tubing in use was labeled with a date of 9/27/25 on two occasions in January 2026. The oxygen concentrator also had no external filter present in the filter compartment. A licensed practical nurse confirmed the tubing date and stated it should be changed weekly, and also confirmed the missing filter should have been in place. For another resident with CHF, dyspnea, hypoxemia, and other unspecified lung disorders, the resident had an order for BiPAP with sleep and PRN. During observation, the resident’s BiPAP mask and attached tubing were found on the floor underneath the bedside locker. A licensed practical nurse confirmed the mask and tubing were on the floor and stated they should have been stored in a bag. The interim DON also confirmed that having the BiPAP mask on the floor was a breach of infection control.
