Walk-in freezer had ongoing ice buildup and condensation over stored frozen food

Summary

The facility failed to effectively clean and maintain food service equipment, specifically the walk-in freezer, affecting 87 residents. During the initial kitchen tour, the Food Service Director reported being in the role for about four months, and the surveyor observed several areas of ice dams appearing to come from the ceiling unit directly over open and closed cardboard boxes of frozen food items, including individual ice creams. Several ice cream containers had solid ice on the packaging, the ceiling had frozen condensation covering a large area, and there was a solid area of ice on the floor just inside the freezer door. The Food Service Director stated the freezer had a history of issues and that maintenance staff monitored it routinely. On a later observation, continued ice buildup was seen on individual ice cream containers with thin paper tops and on several boxes with frozen ice buildup located under the freezer unit. The Maintenance Director reported a long history of ice buildup in the kitchen walk-in freezer with about monthly removal of ice buildup. The Maintenance Director also stated that after ownership changed, maintenance was completed on the unit in the freezer with improvements, but ice dam buildups continued, possibly because staff moved curtains. The Maintenance Director further reported the walk-in freezer had been repaired in 10/2025 prior to the new company purchase.
