On 01/15/2026 at 2:00 PM, surveyors observed cobwebs on the vent cover in the restrooms for rooms 11, 12, and 13. Record review of the Healthcare Services Group housekeeping schedule showed the cleaning resident rooms process includes a 5 and 7 step cleaning process for horizontal surfaces, vertical surfaces, dust mopping, and damp mopping. On 01/05/2025 at 2:00 PM, surveyors observed in room [ROOM NUMBER]'s restroom that a ceiling tile was sagging and partially off the ceiling grid tracks, leaving an accessible space of half an inch between the tile and the track. On 01/05/2026 at 3:15 PM, Maintenance CC stated they were unaware of the sagging ceiling tile in room [ROOM NUMBER]'s restroom.
