Unsanitary Food Handling and Thermometer Use During Tray Line Service

Summary

The facility failed to prepare and serve food under sanitary conditions during meal service. Review of facility policy showed food handlers were required to perform hand hygiene before contact with or preparation of food items and beverages, and employees were required to have hair covered with a hair net and beards and mustaches kept close-cropped. During observation on 03/04/2026, the Dietary Manager was in the kitchen without a beard restraint at 9:01 AM and again at 11:26 AM while the meal tray line was being observed. During tray line service, multiple breaks in sanitary technique were observed. Dietary staff donned gloves and then touched non-food items, left the tray line to retrieve items from elsewhere in the kitchen and the bistro area, and returned without performing hand hygiene or changing gloves. One staff member was observed standing with gloved hands resting against clothing. The food thermometer was also handled unsafely: it was wiped with alcohol, then closed with a gloved hand that contacted the probe area, left open on a temperature log binder with the probe touching the binder, and later set open on an oven mitt with the probe in contact with the mitt before being used again without re-wiping or sanitizing the probe. Staff interviews confirmed that gloves should be removed and hands washed after touching non-food items or leaving the tray line, and that the thermometer should be closed and stored between uses.
