Summary

The facility failed to actively assist and plan for the discharge of one resident who wanted to leave the facility and return to the community. Resident #43 was admitted for skilled care after a stroke and had diagnoses including stroke, depression, and hemiplegia. His MDS documented intact cognition with a BIMS score of 14 out of 15, independence with wheelchair mobility, eating, oral hygiene, toileting, dressing, transfers, and personal hygiene, and partial to moderate assistance only for bathing and shower transfers. The admission MDS and care plan identified his overall goal as discharge to the community and return home. The record showed multiple discussions about discharge-related barriers and possible housing options, including a meeting with therapy, social services, the resident, his friend, and his wife. During that meeting, staff discussed that he was working at the wheelchair level because of hip pain, that he had a vascular appointment, and that a previously found apartment would not work because it had stairs. The SSD documented calls to apartment complexes, a declined referral to another facility due to behaviors, and ongoing discussion of waiver programs and financial barriers. The Administrator later stated specific documentation of discharge work could not be found, and the resident’s Emergency Contact stated he had tried to help but had not been able to find accessible housing and believed the resident should have been discharged much earlier.
