Summary

The facility failed to ensure the kitchen was clean and in good repair during two observations of the dietary area. On the initial tour, surveyors observed frozen french fries on the floor in the walk-in freezer, dirt and streaks on the outside of six bins holding dry goods, yellow crumbs and heavy grease streaks on the deep fryer, orange-brown spills inside the left oven and inside the hot box, soap suds covering an area of the floor under the 3-compartment sink, white and orange spots on the shelf under the dishwasher, a faucet dripping above the garbage disposal, lime and food particles on top of the dishwasher, and heavy dark brown dried grease inside and beneath the convection oven doors. During the second observation, several of the same sanitation concerns remained, including a dripping faucet at the 2-compartment sink, heavy white and brown streaks inside the 3-compartment sink that were greasy to the touch, very heavy black crust on the grill grates, brown-orange streaks running down the wall behind the grill, and heavy brown grease on the steel shelf below the hood vents above the grill and stove. The Dietary Manager stated that Maintenance had fixed the 3-compartment sink faucet before, but he had not notified them of it or the 2-compartment sink faucets leaking. The facility’s checklists showed some cleaning tasks were documented as completed, including wiping out the hot box daily, cleaning hood vents, scraping grill grates, wiping the wall behind the fryer and oven, cleaning compartment sinks, and wiping out ovens.
