Summary

A functional resident call system was not maintained in the resident bathroom and bathing areas, affecting Resident 7 and multiple rooms observed during the survey. On 3/2/26 at 10:11 a.m., Resident 7 repeatedly called out for help, and her call light was not illuminated. During an interview at 10:13 a.m., she stated her call light had not been working for months, and when she pressed the button, the light did not illuminate outside her room. At 10:32 a.m., the Maintenance Director began working on the call light repair, and Resident 7 was given a small hand-held bell in the meantime. Additional observations during the survey identified missing hallway call light covers for rooms 118, 117, 115, 114, 112, and 121, which made the call lights difficult to see if they were on or not. On 3/4/26 at 10:00 a.m., Resident 7's call light was illuminated and the alarm sounded, and an unidentified nurse stated that someone was in the room with Resident 7 but the call light was malfunctioning and would not turn off. During interviews, the Maintenance Director stated he had tried replacing the cord, the room wall-box, the above-the-door box, and even a fuse, but the light still was not working consistently, and he described the system as very old and constantly having problems. The Administrator later stated the call light system was very old and nearly obsolete, with repair parts hard to find and expensive. Resident 7 had also filed a grievance dated 12/19/25 stating she pushed the call light several times but it was not answered in a timely manner.
