Kitchen sanitation and dishwashing failures

Summary

The facility failed to ensure general cleanliness, proper sanitation, and infection control practices during food preparation and dishwashing activities. During an initial kitchen tour, surveyors observed sticky floors in and around the dishwashing area, food crumbs and debris scattered throughout the kitchen, several dented cans still on the service rack, and the dishwashing table where clean dishes were air drying covered with standing water and soggy food crumbs. Temperature logs for the walk-in refrigerator were not readily accessible, and dishwasher observations showed the machine was not reaching the required 180 degrees Fahrenheit. The Kitchen Manager stated she did not know why, and there were no readily accessible dishwasher temperature logs for February; January logs also had missing temperatures, with wash temperatures only reaching 160 and rinse temperatures only as high as 122. During a later observation of the pureed food process, the Kitchen Manager stated the lunch pureed foods had already been prepared and told staff to "Just puree something to get it over with." The kitchen floor again had food crumbs and sticky areas, and standing water was present throughout the dish area, including where clean dishes were stacked. A staff member wore blue rubber gloves, did not wash hands before food preparation, and continued using previously worn gloves while preparing pureed chicken without following a standardized recipe, adding unmeasured water and thickener. The staff member wiped her face after puree splash back and touched other surfaces without hand hygiene before continuing. In the dish room, the staff member and Kitchen Manager ran equipment through the dishwasher, but the staff member did not know how to check PPM, and the Kitchen Manager read the PPM using a test strip placed on a puddle of water. The sanitizer container under the machine was empty, and the Kitchen Manager stated that if the machine was run on a lower temperature with chemical sanitizer, staff should ensure the sanitizer bottle was full and/or functional.
