Summary

The facility failed to ensure ADL care was provided to residents who needed assistance with personal hygiene, specifically shaving and fingernail care, for 2 of 28 residents reviewed. Resident 6 had unspecified dementia, was assessed as having severe cognitive impairment, and was dependent on staff for bathing. During multiple observations from 3/2/26 through 3/5/26, the resident was seen in bed or in a chair with untrimmed facial hair around the mouth and long, untrimmed fingernails with dark debris underneath them. During one observation, the resident stated she did not like having long fingernails. Resident 6's record showed an ADL self-care performance deficit and indicated she was totally dependent on one to two staff members for personal hygiene. However, progress notes, behavior documentation, and shower sheets from February and March 2026 did not document that shaving or fingernail care was offered, provided, or refused. One shower sheet noted the fingernails were cleaned but not trimmed or filed, and there was no documentation that shaving was offered or completed. A CNA stated Resident 6 did not refuse care and that fingernail care and shaving should have been completed with showers. Resident 15, who had legal blindness and moderate cognitive impairment, was observed on multiple occasions with long, untrimmed fingernails and dark debris underneath them. The resident stated he could not safely care for his own fingernails and would have allowed staff to provide the care if offered, but staff had not offered to clean or trim them. His record indicated he required set-up or clean-up assistance with personal hygiene and had an ADL self-care performance deficit, but shower sheets and progress notes lacked documentation that fingernail care was offered, provided, or refused. The DON stated refusals should have been documented in CNA charting and re-attempts documented in progress notes.
