Summary

The facility failed to ensure a new PASRR level one was completed after a new major mental illness diagnosis was added to the diagnosis list for two residents reviewed for PASRR compliance. One resident, whose diagnoses included dementia, major depressive disorder, and psychotic disorder, had psychotic disorder added to the medical chart on 5/12/25, but the record lacked documentation that the facility submitted a new level one PASRR to include that diagnosis. The facility policy stated that a negative level 1 screen ends the PASARR process unless a possible serious mental disorder arises later, and that any resident with newly evident or possible serious mental disorder must be referred to the appropriate state-designated mental health authority for review.
