Summary

The facility failed to ensure a resident with legal blindness received the adaptive equipment identified in the care plan for meals and daily use. The resident’s care plan, dated 1/31/24, documented an ADL self-care performance deficit related to impaired eyesight and directed staff to provide a divided plate, Kennedy cup, and blue placemat for all meals. During observations, the resident was seen at the nurse’s station stating he was tired of waiting and wanted his lunch, and he believed it was 2:00 p.m. when staff told him it was around noon. In his room, a clock next to his bed was observed to be set about two hours fast, and the resident stated the clock announced the time each hour so he could know what time it was, but he was not aware it was incorrect. The resident was also observed receiving lunch without the adaptive items listed in the care plan. On one observation, lunch was served in Styrofoam containers with no adaptive equipment provided. On another observation, the resident’s lunch was served on a regular white plate, with drinks in a small glass without a lid and a Styrofoam cup with a lid and straw, while the table had a white tablecloth. The resident stated he had previously received a divided plate, sippy cups, and a blue placemat, and that these items helped him because he could not see a white plate on a white tablecloth and the placemat helped him locate his plate. The DON stated residents should receive adaptive equipment as indicated in their care plan, and the facility policy stated it provides special eating equipment and assistive devices as deemed necessary.
