Kitchen sanitation and dishwashing temperature failures

Transcendent Healthcare Of BoonvilleBoonville, Indiana Survey Completed on 03-13-2026

Summary

The facility failed to ensure proper sanitation in the kitchen during observations of food service operations. The high temperature dishwasher was observed to run three consecutive times without reaching a final rinse temperature above 73 degrees Fahrenheit during one observation, and later was observed again with rinse temperatures of 169 degrees Fahrenheit, 171 degrees Fahrenheit, and 169 degrees Fahrenheit. The Kitchen Manager stated the dishwasher had not gotten above 180 degrees Fahrenheit during the first observation because staff had just gotten there that morning, and maintenance had looked at it and said it was working fine, though the heating element may have been going out. She also stated staff were not watching the temperature and were just documenting a temperature, so she was unsure how long it had not been at the correct rinsing temperature. During a separate observation, Cook 2 washed hands with a 10 second lather and the Dietary Manager washed hands with a 7 second lather. The Dietary Manager also used a suction cup to pick up clean plates and placed it on the plate holder cart after setting the plates on the plate warmer, rather than placing it on the next clean plate. Food was then served on those plates. The Infection Preventionist stated staff should lather their hands for at least 20 seconds during handwashing, and the Dietary Manager stated the bottom of the suction cup used for picking up clean plates should go on the next clean plate instead of on the cart.

Short Summary

Improper Thawing of Frozen Chicken: Staff thawed frozen breaded chicken in a 3-compartment sink without cold running water instead of using an approved defrosting method. A Cook said she removed the chicken from the freezer about an hour earlier, and the DM confirmed the chicken was thawed in the sink after staff forgot to remove it from refrigeration earlier. The facility's policy required thawing in refrigeration, under running water, in a microwave under specific conditions, or as part of the conventional cooking process.

D
F0812 F812: Procure food from sources approved or considered satisfactory and store, prepare, distribute and serve food in accordance with professional standards.
Short Summary

Ice Machine and Water Filter Not Properly Cleaned: An observation found black and brown substance on the ice machine's water curtain guard and a dark brown water filter behind the machine. The dietary manager was unsure when the ice machine was last cleaned or how often the filter was changed, while the ADM believed it had been checked and cleaned within the prior week. The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
