Food Served Cold and Unpalatable
Summary
The facility failed to ensure residents' food was palatable, attractive, and served at the proper temperature during meal service observations. During an interview with kitchen staff, the dietary staff indicated lunch was to be served starting at 10:50 A.M., but they were unaware they needed to check steam table temperatures before serving and stated they were the only staff working on the weekend. During a kitchen tour, the lunch meal of chicken ala king over toast, broccoli, and mashed potatoes was observed for residents on a pureed diet, and the pureed broccoli and mechanical soft chicken did not reach an acceptable temperature, leading staff to place the metal steam table containers on the gas stove to heat them up. A test tray sampled during observation was described as cold to taste, bland, with no flavor, and with carrots that were hard and difficult to chew. Resident interviews also described repeated concerns with cold and unappealing food. One resident stated the food was not appealing and was usually cold when received in the room, while another said the food was bad, always cold, and had to be warmed up by staff even though the resident felt she should not have to ask. Additional residents and a family member reported that food delivered to rooms was sometimes cold and not always good, and one resident said there were a lot of concerns about the food all the time. A later test tray from the hall cart showed regular spinach at 113 degrees Fahrenheit, pureed spinach at 110 degrees Fahrenheit, and pureed ham and beans at 110 degrees Fahrenheit, and the Regional Dietary Manager stated food on the steam table should not be served below 135 degrees Fahrenheit. The facility policy stated food should be prepared, held, and served to preserve nutritive value and palatability, with hot foods served hot and cold foods served cold.
