The census at the time of the survey was documented as 143 residents, and the deficiency was identified through review of resident council minutes, grievance records, and resident interviews, which collectively demonstrated a pattern of concerns about food temperature despite the existence of a policy requiring food to be served at safe and appetizing temperatures.
