Summary

The facility failed to provide the services of a Registered Nurse (RN) on duty for at least eight hours each day, as required. Review of the facility’s daily staffing schedules for the period from 1/1/2026 through 2/28/2026 showed that no RN worked at least eight hours on multiple specific dates: 1/4, 1/10, 1/11, 1/17, 1/18, 1/24, 1/25, 1/31, 2/1, 2/7, 2/8, 2/14, 2/15, 2/21, 2/22, and 2/28 of 2026.
