Summary

The facility failed to adequately protect a vulnerable resident from sexual abuse by another resident with a documented history of sexually inappropriate behavior toward staff and other residents. On 03/17/2026, facility staff observed the cognitively impaired resident in bed with the other resident on top of her, and the resident was not wearing a brief at the time. The resident was later sent to the hospital for a rape kit and STD testing as a precaution, and the hospital summary described the admission as medical clearance following a suspected sexual battery. The resident involved had severe cognitive impairment, with a BIMS score of 3, and diagnoses that included Alzheimer's disease, psychosis, cerebral infarction, depressive episode, and mood disorder.
