The resident was admitted with muscle weakness and difficulty walking, and her MDS indicated she wore glasses and had a BIMS score of 10/15, reflecting moderate cognitive impairment. During an observation and interview, the resident’s eyeglasses were noted to be covered in smears, and she reported that she had informed staff that her glasses needed to be cleaned but no one had done so, making it hard for her to see. The facility’s Resident Rights policy, revised October 2025, stated that residents are guaranteed a dignified existence, and the DON stated that her expectation was that staff clean residents’ eyeglasses daily. This failure could have resulted in a decrease in the resident’s vision and emotional well-being.
