During the Entrance Conference on 2/23/2026, the Assistant Director of Nursing (ADON) was given the Entrance Conference Worksheet, which included a request for the QAPI Plan. On 2/26/2026, during a concurrent interview and record review with the ADON, the survey binder and Entrance Conference Worksheet were reviewed, and the ADON stated the QAPI Plan in the binder was not the correct one and was dated 1/26/2023. The ADON stated she did not have access to the current QAPI Plan, and also stated the Administrator was unable to be reached and the Director of Nursing was not available. The ADON stated the QAPI Plan is a written plan used to identify problem care areas, discuss and fix them, and that it contains data, logs, in-services, meeting minutes, and analysis during QAPI meetings by the DON. The facility’s policy titled Quality Assurance Performance Improvement (QAPI), last reviewed 10/23/2025, stated the facility maintains documentation and demonstrates evidence of its ongoing QAPI program and that the QAPI plan is a written plan describing the process for conducting QAPI/QAA activities. The report also states the facility failed to provide documentation of data collection and analysis at regular intervals for falls, which the facility had identified as a problem issue.
