Dietary Staff Lacked IDDSI Competency for Pureed Foods
Summary
The facility failed to provide sufficient support personnel with the appropriate skills and competencies to carry out food and nutrition services when kitchen staff did not demonstrate knowledge of or competency with IDDSI testing for mechanically altered diets. On 2/23/2026, during tray line, the Dietary Supervisor, a cook, and other dietary staff prepared a puree meal for Resident 25 without performing IDDSI testing on the puree foods. The resident’s lunch tray included puree broccoli, puree pasta, and puree bread, and the broccoli was observed with a green-tinted, water-like liquid surrounding it. The staff member plating the meal stated the broccoli was separating and should not have water, and then stirred and replated the meal. Resident 25’s record showed admission on 11/6/2025 with diagnoses including dysphagia, chronic respiratory failure with hypoxia, and COPD. The resident’s MDS dated 2/4/2026 indicated the resident required supervision or touching assistance with eating and oral hygiene and was on a mechanically altered and therapeutic diet. The physician’s order summary dated 2/26/2026 showed an order for a consistent carbohydrate, no added salt, pureed texture, mildly thick consistency, fortified diet. The facility’s lunch menu for that day listed pureed seasoned fresh broccoli florets as part of the pureed meal. During interviews and record review, the Dietary Supervisor, the cook, and another dietary staff member stated they were not familiar with the term IDDSI spoon tilt test. One staff member stated the broccoli was prepared by placing an unmeasured amount of cooked broccoli and an unmeasured amount of cooking water into the blender. The Dietary Supervisor stated the facility had implemented the menu and recipe system with IDDSI about a year earlier, but no in-service had been provided to kitchen staff and no competency evaluations had been completed for IDDSI testing. The Registered Dietitian stated he was responsible for making sure kitchen staff were competent, but he had not provided an in-service or competency evaluation for the Dietary Supervisor. The Speech-Language Pathologist stated puree level 4 should be like a pudding texture and should pass visual checks, the spoon tilt test, and the fork drip test. The Assistant Director of Nursing and Governing Body Chair both stated the staff did not demonstrate competency with IDDSI standards, and the Governing Body Chair stated there was no documentation that the Administrator provided competency evaluations for the Dietary Supervisor.
