Summary

The facility failed to maintain an effective pest control program as evidenced by unresolved structural issues and inadequate sanitation contributing to the presence of pests. A pest control service report dated 3/4/2026 documented that the baseboard on the right side of the entry door was peeling and damaged, creating a potential entry point and harborage area for roaches, and recommended sealing the peeling baseboards and maintaining high sanitation standards, including preventing food particles and water leaks around kitchen appliances and in patient rooms.
