Summary

The facility failed to store, prepare, distribute, and serve food in a safe and sanitary manner. During observation on 03/02/2026, the grease trap under the griddle was found to contain approximately two inches of black sludge and debris, and the Dietary Manager stated it should be cleaned weekly but could not say when it had last been cleaned because it was not listed on the cleaning schedule. A review of the kitchen cleaning schedules from 02/09/2026 to 03/01/2026 showed no specific cleaning tasks for grease traps, and the facility’s cleaning policies did not include specific instructions or frequency for drip pans and grease traps. The Administrator stated grease traps should be cleaned daily or frequently depending on use, and staff acknowledged that dirty grease traps were a fire hazard and could lead to pest issues. The facility also failed to prevent cross-contamination in the food preparation area and failed to avoid bare-hand contact with ready-to-eat food. During observation, a personal cellphone belonging to staff member #2 was seen on a food prep counter, and staff confirmed personal items were to be kept out of the kitchen because of contamination risk. In a separate observation, a CNA delivered a meal tray to a resident, opened a package of crackers, removed the crackers with bare hands, and placed them directly into the resident’s bowl of chili; the CNA stated the belief that bare-hand contact with food was acceptable if hand sanitizer was used between residents. The facility’s hand hygiene policy did not address specific requirements for food service or prohibit bare-hand contact with ready-to-eat food, although the cooking guidelines stated staff should wash hands before handling foods and wear gloves when handling all foods.
