Summary

The facility failed to provide specialized rehabilitative services for one resident with a history of CVA, left hemiparesis, wheelchair dependence, and assistance needs for multiple ADLs. The resident’s comprehensive care plan identified her as at risk for falls, contractures, and decline in overall wellbeing, and included therapy to evaluate and screen as ordered and as needed. Despite repeated provider progress notes stating to continue PT and a later note referencing PT evaluation, the resident reported she had not had any therapy since admission and wanted to walk again and be more independent. Nursing staff stated therapy had not provided exercises, and the rehab director stated the resident had been evaluated by therapy but the evaluation was not approved by insurance, so she was not in therapy. The rehab director also stated the resident had not had therapy in the last year due to insurance non-approval, and the administrator stated that if therapy was denied or there was no payor source, they were to be notified so the resident’s needs could be addressed.
