Summary

The facility failed to ensure appropriate perineal care for a male resident who was frequently incontinent of bowel and bladder and required substantial to maximum assistance with toileting and personal hygiene. The resident had diagnoses including diabetes, CVA, and hemiplegia, and his quarterly MDS reflected intact cognition with a BIMS score of 15. During an observation, CNA C provided incontinence care but wiped the penis and scrotum toward the anal area, then cleaned the anal area outward, and placed a clean brief without cleaning the pubic area, penile shaft, or scrotum. During interview, CNA C stated she thought she had performed peri-care correctly because she wiped from front to back, but she did not know she was supposed to hold the penis and wipe the penile shaft toward the scrotum. She also stated she did not clean the pubic area. Her skills checklist reflected competency in infection control, peri-care, and catheter care. The facility policy for male perineal care required cleansing the tip of the penis, the shaft using downward strokes toward the scrotum, and cleaning the scrotum and anal area with separate wipes. The facility also failed to ensure appropriate catheter and perineal care for a female resident who was always incontinent of bowel and bladder and had a Foley catheter. Her diagnoses included Parkinson’s disease, dementia, and diabetes, and her admission note stated she had been readmitted for a kidney infection and had a light pink Foley drainage. During observation, CNA B wiped across the pubic area and down each side of the groin but did not separate the labia or wipe down the middle, and did not clean the catheter tubing from the insertion site downward. CNA B stated she knew she did not go down the middle or wipe down the catheter tube and identified the risk as urinary tract infections. The facility policy for catheter care required separating the labia and wiping the catheter from the insertion site downward.
