Care plans omitted contracture interventions for two residents
Summary
The facility failed to ensure the comprehensive care plan included services to meet the needs of two residents with contractures and to maintain or improve range of motion. For Resident #17, the quarterly MDS reflected intact cognition, substantial to maximum assistance with personal care, bowel and bladder incontinence, and functional limitations in range of motion on one side related to diabetes, CVA, and hemiplegia. His care plan addressed CVA and fall risk, but it did not include his left wrist contracture or interventions to prevent further decline, even though the record showed he had not received OT or restorative nursing services in the look-back period. Record review showed a skilled therapy referral for Resident #17 initiated by the Director of Rehab for a hand contracture, with a recommendation for a left resting hand splint to be worn for a maximum of 4 hours and during breakfast and lunch to support self-feeding. During observation, he was lying in bed with limited use of his left hand and his wrist turned downward about 90 degrees, and he stated he had been trying to get a splint for months and did not want the wrist to get worse. OT later assessed him and stated he would benefit from therapy to improve flexibility and decrease pain and from a resting hand splint. For Resident #89, the quarterly MDS reflected intact cognition, dependence on staff for personal care and eating, bowel and bladder incontinence, and functional limitations in range of motion of both upper extremities and one lower extremity related to cervical spinal stenosis and COPD. His care plan noted assistance with self-care and mobility, but it did not include interventions for contractures of the right arm, left hand, or shoulder. A skilled therapy referral also recommended a left resting hand splint for use during meals. During observation, he was lying in bed being fed lunch, with his right arm drawn up to his neck and his left hand contracted; he stated he wanted something done for his right hand to improve independence and said he had not had exercise for his left hand. OT assessed his left hand, right hand, and right arm and stated he would benefit from therapy to improve flexibility and decrease pain and from a resting hand splint.
