A Certified Occupational Therapist Assistant said she was a certified activity director but had only volunteered to call bingo and knew nothing about assessment requirements or documenting activities. The Administrator said he expected activity assessments to be completed quarterly and acknowledged that the Activity Director had been hired without certification. The facility policy stated that the Activity Director determines the need for individual programming through the resident assessment process and that the Activity Director and staff assess residents’ ability or interest at least quarterly, but the policy did not list required qualifications for the Activity Director.
